Responsibilities:

Job Summary: Performs duties essentially of personal care which might be given to convalescent, chronically ill or disabled persons in their homes.  Attempts in every possible way, in working with individuals and families, to assist them to assume responsibilities toward self care.  Helps the well family members to carry their share of the responsibilities and encourages the disabled member to participate in the life of the family, in so far as able.

Qualifications/Educational Requirements: 

  • Graduate of an accredited High School or equivalent preferred.
  • Completion of approved Certified Nurse Assistant course.
  • Certified by State Department of Health Service.
  • Work positively and favorably with patients, families, and staff.
  • Demonstrate compassion, responsibility, and cheerful attitude.
  • Listed on the nurse aide registry with no finding against the aide relating to client abuse or neglect or misappropriation of client property.
  • Meets all agency clinical requirements

Responsibilities/essential functions: The person in this position must be able to perform the following essential job functions with or without reasonable accommodations.

  • Follow the instructions, verbal or written, of the professional nurse/therapist in providing care.*
  • Possess adequate computer skills to accommodate electronic charting. *
  • Provide assistance with hygiene such as bathing, oral care, and dressing.*
  • Perform and record accurate measurements (i.e. vital signs, or intake/output as instructed in the care plans). *
  • Observe and report any safety hazards found in the client’s home or any significant observations regarding the client.*
  • Attend staff meetings, attend/complete 12 hours of annual in-services, and participates in orientation of new employees. *
  • Report patient complaints to the RN. *
  • Maintain patient confidentiality/adheres to HIPPA requirements and agency policy and procedures manual. *
  • Report abnormal findings in patient’s conditions as observed and per care plan to RN. *
  • Attends annual review
