The Physical Therapist is responsible for the assessment of referred patients, ranging in age from 18 years to geriatric who are experiencing medical conditions requiring rehabilitation and/or wound care needs.

The Physical Therapist duties include:

  • Responsible to provide direct patient care to assess their medical condition, functional capabilities and limitations and restrictions and potential for rehabilitation..
  • Documents. prepares/reviews and completes plans of care, progress notes, order and other clinical record documentation.
  • To establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician.
  • Orders, assist, instructs patient and family in transfer techniques and equipment specific to patient diagnosis and needs
  • Educates patients and families in appropriate physical therapy methods.
  • Provides patient and family education regarding procedures based on patient;s age, condition, and readiness
  • Oversees technicians, aids, etc
  • Perform duties consistent with patients needs and desires
  • applies concepts of infection control and precaution in coordinating, performing patient care activities for prevention purposes.
  • Performs additional duties as assigned.


Minimum Qualifications:

  • Ability to project a professional image
  • Knowledge of regulatory standards and compliance requirements
  • Knowledge and experience in the needs of patients ranging in age from 18 to geriatric
  • Working knowledge and ability to apply professional standards of practice in job situations
  • Strong organizational and analytical skills; excellent writing and speaking skills
  • Working knowledge of personal computer and software applications used in job functions (Word processing, graphics, databases, spreadsheets, etc.)
  • Persons who have been found guilty by a court of law of abusing, neglecting, or mistreating individuals in a health care related setting are ineligible for employment in this position
  • Freedom from illegal use of drugs
  • Freedom from use of and effects of use of drugs and alcohol in the workplace
Education and Experience:
  • Graduate of an accredited school for Physical Therapy. 
  • At least six months of experience directly related to duties and responsibilities. 

Certificates, Licenses, Registrations: 

  • Current license for Physical Therapist in state of employment. 
  • Must maintain current Provider CPR certification throughout employment.
