Responsibilities

The Clinical Social Worker/Licensed Mental Health Professional is responsible for assessment of patient/family psychosocial needs, planning, implementing, procuring services for and evaluating outcomes for geriatric patients in a behavioral healthcare setting. Responsibilities include providing psychosocial assessment, discharge planning, services coordination along the continuum of care and counseling to patients and families.  Participates in functions to coordinate the care of patients with all health team members throughout the facility.  Integrates Social Services within the hospital’s primary functions in a manner that contributes to the provision of comprehensive health care and achievement of quality cost effective outcomes.

  • Leads the team on discharge planning process ensuring that each patient has a safe and appropriate discharge plan to achieve positive outcomes
  • Develops plan of care with specific goals, objectives, and interventions defining actions unique to each patient’s needs. Updates and revises plans as goals and objectives are met or the patient’s condition changes
  • Understands the age differences and developmental needs of patients when developing care plans
  • Participates in educational programs and in-service meetings, Performance Improvement, policy and program development, and other committee meetings when requested
  • Maintains current list of all community referral agencies and services. Distributes resource materials to patients and families and arranges community referrals as needed
  • Serves as a clinical resource person for team members
  • Responds to patient and family concern, ensures that each concern is documented and a resolution is initiated
  • Supports staff, patient and family education to enhance knowledge, skills and necessary behaviors to facilitate positive outcomes
  • Promote the philosophy, mission and objectives of the Company within and to customers outside the hospital
  • Demonstrates thorough understanding of local, state and federal regulations, accrediting agency standards and NASW Standards and Code of Ethics
  • Demonstrates honesty and integrity at all times in care and use of patient and hospital property
  • Demonstrates ability to prioritize tasks/responsibilities and complete duties within allotted time
  • Assists with involuntary admission paperwork and protocols according to hospital policies and state and local laws
  • Provides group, individual and family therapy to meet the patient’s needs and in accordance with the patient’s treatment plan
  • Completes all documentation in a thorough and timely manner including progress notes, assessments, treatment plans, discharge instructions and summary
  • Participates in meetings and committees as requested
  • Identifies and reports victims and perpetrators of abuse according to established hospital standards, state and local laws
  • Comply with HIPAA regulatory requirements
  • Perform other duties as assigned

Minimum Qualifications:

  • Ability to project a professional image
  • Knowledge of regulatory standards and compliance requirements and hospital policies and procedures
  • Working knowledge and ability to apply professional standards of practice in job situations
  • Strong organizational and analytical skills
  • Working knowledge of computer and software applications used in job functions (order entry, data entry)
  • Persons who have been found guilty by a court of law of abusing, neglecting, or mistreating individuals in a health care related setting are ineligible for employment in this position
  • Freedom from illegal use of drugs
  • Freedom from use of and effects of use of drugs and alcohol in the workplace

 Education and/or Experience:  

  • Master’s degree in social work with a specialization in Clinical Social Work
  • PhD in Clinical Psychology
  • One (1) year experience in all aspects of social services with an emphasis in psychiatry is preferred
